​​Indicator 13: Aligning Transition Planning to Postsecondary Outcomes

Indicator 13 is more than a monitoring requirement — it is a quality indicator for postsecondary readiness.

This session helps teams examine how age-appropriate assessment, measurable postsecondary goals, annual goals, transition services, and course of study work together as a coherent system. Participants will explore how transition planning connects to graduation pathways, career readiness, and long-term student outcomes.

Outcomes include:

  • Clear understanding of federal and state Indicator 13 requirements
  • Improved alignment between assessment, goals, and services
  • Reduced compliance risk through proactive documentation practices
  • Stronger student-centered transition planning

Teams will leave prepared to strengthen transition quality across their district.
